Use this board survey questionnaire as part of evaluating your board. It acts as a self-assessment tool, encouraging board members to reflect on their performance and contributions to your organisation. 

Board members should complete the questionnaire individually, usually in confidence, and provide feedback on the board’s overall performance.  

You can combine responses to understand perceptions of board effectiveness and find opportunities for improvement.
